Job Description :
At Rehoboth Home Care a Licensed Practical Nurse (LPN) provides direct nursing care to clients in their homes under the supervision of a Registered Nurse or physician. The LPN supports patient health and well-being by delivering compassionate, safe, and effective nursing services while promoting comfort, dignity, and independence.
Duties & Responsibilities:
- Provide direct patient care in accordance with the plan of care
- Administer medications and treatments as prescribed
- Monitor vital signs and observe changes in patient condition
- Perform wound care, catheter care, and other skilled nursing procedures
- Collect and document patient data accurately and timely
- Adhere to agency policies, procedures, and state nursing regulations
Minimum Qualifications:
- Valid and active Licensed Practical Nurse (LPN) license
- Graduate of an accredited practical nursing program
- Minimum of 1 year of nursing experience (homecare experience preferred)
- Excellent communication and interpersonal abilities
- Ability to work independently and manage time effectively
- Compassionate, patient-centered approach to care
